We partner with Solent NHS Trust to employ a First Contact Physiotherapist. This new service provides same-day telephone appointments for musculoskeletal conditions, as well as face-to-face sessions in our surgeries.

We partner with TheYouTrust to employ two Social Prescribing Link Workers and two Health and Wellbeing Coaches.

Our Social Prescribing Link workers take a holistic approach to people’s health and wellbeing, connecting patients to community groups and statutory services for practical and emotional support. They also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.

Our Health and Wellbeing Coaches are trained in Health Coaching and can share insights that can help people change their mindset. They can help with changes related to your health and wellbeing whether that’s through healthy eating, physical activity, sleep, stress/anxiety/low mood or managing long term conditions.

Our Primary Care Network has worked together with the other two Primary Care Networks in Havant Borough to deliver the COVID Vaccination Programme.

Primary care networks (PCNs) are a key feature of the NHS Long Term Plan, supporting the development of proactive, personalised and coordinated health and social care services.

Health and care professionals often describe Primary Care Networks (PCNs) as a change from reactively providing appointments for patients to proactively caring for the people and communities they serve. Patients are not likely to notice much of a difference at first to the care they receive, but the accessibility to health care services available and breadth of health care professional services available are the key difference. .

A key feature of practices coming together as part of a primary care network means that every patient registered at those practices will now be able to access some care differently.

The formation of our Primary Care Network (PCN) brings considerable funding to primary care services across the Havant borough, enabling us to employ a range of professionals to improve the health and wellbeing of our Hayling Island and Emsworth population.

We have 2 Clinical Pharmacists and a Pharmacy Technician. They will support all practices in our network and are highly qualified experts in medicines that can help people in a range of different ways. This includes carrying out structured medication reviews for patients with on-going health problems and improving patient safety, outcomes and value through a person-centred approach.

The introduction of Clinical Pharmacists in each network will support GPs to focus their skills where they are most needed, such as diagnosing and treating patients with more complex conditions. This will ultimately help GPs to manage the demands on their time.

The COVID Vaccination Programme across Havant Borough has now delivered over 90,000 vaccinations. 32,500 at Emsworth Baptist Church and 20,450 at Hayling Health Centre. When we read the evidence about how it has successfully reduced transmission, as well as reduced hospitalisation and death we should all feel very proud of the huge contribution we have made.

It has been an amazing achievement and one our communities have been very grateful for. These have not just been at our sites, but in all our care homes and housebound patients too. Our rates are within the top 20 CCGs in the country and no parts of our population across the Havant Borough have been left behind. The rewards are in the lives saved, falling rates and the rise in community spirit and appreciation. It has been achieved through hard work, focus, sheer determination and collaboration. However, it is important to recognise it has also been an immensely challenging time, hugely disruptive and asked us to ask more of our teams when life is hard, with barely anything functioning normally. Though we know it’s worthwhile, I also know we now face the daunting task of second doses, finishing off the first doses and continuing to keep primary care open for business. All while blending this with our non-working lives, which we are desperate to claw back to some sort of normality. Achieving and maintaining the right pace feels like a tight rope, but I have been personally grateful for honesty, forgiveness and smiles. So much of this Covid Vaccination Programme has been out of our control, yet we have achieved so much together working with two neighbouring PCNs. It’s been a tough Winter, but the outcomes from our vaccination clinics have been impressive. Total team effort in the heart of our communities. Thank you!
