We are pleased to introduce our Social Prescribing Link Workers, Clare and Sarah, for Hayling Island and Emsworth PCN. Our dedicated team takes a holistic approach to health and wellbeing, supporting individuals by connecting them to a wide range of community groups, local services, and statutory support for both practical and emotional needs.
In addition to helping individuals, our Social Prescribers work closely with existing community groups to ensure they remain accessible and sustainable, while also assisting in the creation of new initiatives.
