General Practitioners (GPs)

GPs from Emsworth Practice, The Elms Practice and Waterside Medical Practice are highly qualified to support our patient. They treat all common medical conditions and refer patients to hospitals and other medical services for urgent and specialist treatment. They focus on the health of patients within our communities by combining physical, psychological and social aspects of care. They are experienced in diagnosing and treating patients with complex conditions.

First Contact Physiotherapist

We partner with Solent NHS Trust to provide this new service, offering same-day telephone appointments for musculoskeletal conditions, as well as face-to-face sessions in our surgeries.

Social Prescribing Link Workers

Our Social Prescribing Link workers take a holistic approach to people’s health and wellbeing, connecting patients to community groups and statutory services for practical and emotional support. They also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners. We are proud to partner with the YOU Trust to facilitate this new service.

Clinical Pharmacists and a Pharmacy Technician

Supporting all practices in our network, our Clinical Pharmacists and Pharmacy Technician are highly qualified experts in medicines that can help people in a range of different ways. This includes carrying out structured medication reviews for patients with on-going health problems and improving patient safety, outcomes and value through a person-centred approach. Clinical Pharmacists help to support GPs to focus their skills where they are most needed, such as diagnosing and treating patients with more complex conditions.

Practice Nurses and Paramedic

Our Practice Nurses and Paramedic are highly trained and a vital piece of our PCN jigsaw. They work tirelessly to support all aspects of primary care services within our PCN to support patient consultations within each surgery. This includes physical examinations, investigatory procedures and routine tests. They also work with our GPs and other primary care professionals in diagnosing and treating illnesses and ailments as well as treating wounds and carrying out routine procedures.

Care Coordinators

Working within our Hayling Island and Emsworth communities, our Care Coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They work closely with our GPs and other primary care professionals within the PCN to make sure that appropriate support is made available to patients and their carers, and ensuring that their changing needs are addressed.

Volunteers

We are so grateful for the ongoing support that we receive from our valued volunteers. Like our team, most of our volunteers live in the local community and generously donate their time and skills to help us to achieve targets that help us to provide the best services we possibly can to our patient communities. Recent volunteer contributions include gardening and landscaping, fundraising for a community trike and significant community volunteer support in carrying out COVID 19 vaccination clinics.
